Self-referral allows patients to access certain NHS health services without needing an appointment with their GP. Instead of going through their surgery, patients can directly refer themselves to specific services.

A Self-referral can help you find the care you need more quickly and easily and empowers you to take control of your health journey.

MSK Physio

As a team we aim to promote, restore and maintain your physical capabilities, function and wellbeing. We provide specific advice and treatment to help you improve your function, return to your activities, and improve and maintain your quality of life.

Here at NHFT our physiotherapists are highly skilled, independent practitioners. Each specialises in their own areas of further learning and specific skill sets.

Musculoskeletal Physiotherapy relates to physiotherapy for the injured and/or problematic areas of the muscular and skeletal (MSK) system. This includes:

  • Muscles and Tendons
  • Joints, Ligaments and Cartilage
  • Fractures / Broken Bones
  • Nerves

Advanced Physiotherapist Practitioners (APP’s) are highly specialised clinicians who work at a higher level of varied advanced practice. Skills include:

  • Diagnostics
  • Injection therapy
  • Independent prescribing
  • Provision of a fit note (Med3)

Who is this service for? Adults and Young people aged 12 years old or older, requiring assessment, diagnosis and treatment for musculoskeletal conditions.

Diabetes and High-Risk Foot Service

We are a team of highly skilled podiatrists who work closely with specialists in rheumatology, diabetes and vascular surgery.

We provide assessment, diagnosis and treatment for predominantly high risk and diabetic patients who meet the criteria for our service. We also support others with high risk foot conditions, who are vulnerable, as well as children and young people under 18 years of age. We offer advice on ways that people can manage their own foot care, choose footwear and information on other support available.

Who is this service for? Patients with diabetes and other high risk foot conditions who are vulnerable, as well as for children and young people under 18 years of age.

Falls- North

The Falls Management Service is a therapy-led service which offers specialist assessment and advice for people with a concern of falling or a recent history of falling.

The service is made up of a team of specialist falls practitioners. It is available for people living in North Northamptonshire who require support with reducing their risk of falling.

Wheelchairs

Includes wheelchair and special seating assessments for children and adults with mobility and postural needs. We also provide pressure care cushions and accessories for wheelchairs and buggies.

We have a team of specialist clinicians including Occupational Therapists, rehabilitation engineer technicians and field service engineers along with a team of administrators who are dedicated and committed to delivering a high quality and efficient service.

It is now possible for a patient to start a self-referral for a manual wheelchair through NWOS – provided by Blatchford Limited using the following form: BlatchfordSR (softoptions.co.uk)

This does not include referral for other items such as buggies or specialist wheelchairs.
Powered wheelchair referrals will still require a GP to complete.

Community Equipment

Equipment can be everyday gadgets and specially designed equipment that can help you complete tasks more easily.

Often, they are things you can purchase yourself at little cost, but sometimes they are more specialised and need to be acquired from a specialist supplier.

Types of equipment

There are lots of items you can get to make tasks easier, including:

  • walking aids – like walking sticks, walking frames, or folding walkers with wheels
  • kitchen aids – such as non-slip jar and bottle openers or plate clip-ons to prevent you spilling your dinner
  • dressing aids – like a long-handled shoehorn or leg lifter
  • sitting aids – such as devices that will lift you out of your seat or boost the height of your toilet
